Medication Therapy Management Program (MTMP)
MTMP is an exciting feature available to all qualified Windsor Rx enrollees. The program may assist people that meet the following criteria:
- Multiple chronic diseases such as Diabetes, Coronary Artery Disease (CHD), Asthma, Heart Failure, Osteoarthritis, Rheumatoid Arthritis, Transplant, HIV/AIDS, Hemophilia, Renal Failure.
- At least eight or more prescriptions for three consecutive months.
- Likely to incur annual cost for covered Part D drugs that exceed $4,000.
Why is MTMP important?
MTMP helps identify potential errors and gaps in your care.
MTMP:- Helps reduce the risk of medication errors, especially if you have chronic conditions, take several medications or see multiple doctors and other care providers.
- Provides current information on proven medical practices to help you and your doctor determine the most effective treatment.
- Helps you understand your condition and medications, so you can take an active role in managing your health.
What are the benefits of MTMP for me?
Windsor’s Rx pharmacists and clinicians may communicate with you and your doctor to help you understand your condition and the prescriptions you take, whether they are right for you and if you are taking them according to your doctor’s directions.
We will check all of your medications for potential safety concerns and work with you and your doctor to eliminate drug interactions and therapeutic duplications.
Your MTMP participation may help improve your overall health and reduce your risk for serious medication reactions, hospitalization and emergency room visits. Our program is not considered a pharmacy benefit.
What will MTMP cost me?
There is not extra cost for the program.
How will I know if I’m part of MTMP?
Windsor enrollees are sent a welcome letter and explaining the ability to opt-out of the program if desired. Internal personnel at Windsor perform all MTMP management and processes. This process involves a MTMP Team consisting of pharmacy and case management. The program oversight is the responsibility of the Chief Medical Officer and Pharmacy Director of Windsor.
